land lease recording

i was wondering if anybody has recorded the sugarbush lease agreement a county office in ny? today i went to erie county offices to record the agreement, and they told me that i need form tp-584 and the notary signing wasnt right supposedly. any input would be appreciated.

My leases are recorded by way of the land owners filing for farm assessment. Each has done so, and they need a notorized copy of the written lease for the ag assessment to be processed. I don't remember what forms were used.
